What is a Hydrocele?
A hydrocele forms when fluid accumulates between the two layers of the tunica vaginalis, a thin pouch that holds the testicle. The fluid build-up causes visible, painless swelling of the scrotum.
Types of Hydrocele
Communicating Hydrocele
The sac surrounding the testicle does not close properly, allowing fluid from the abdomen to flow in and out. More common in infants.
Non-Communicating Hydrocele
The sac is closed but the body does not absorb the trapped fluid. Common in adults, often following injury or infection.
Symptoms
- Painless swelling of the scrotum on one or both sides
- Heaviness or discomfort in the scrotum
- The swelling may be larger in the morning and decrease by evening
- Smooth, tense swelling that transilluminates (light passes through)

- Scrotal ultrasound to rule out other causes
Surgical Treatment – Hydrocelectomy
Hydrocelectomy is the standard surgical procedure to treat a hydrocele. Under general or spinal anesthesia, an incision is made in the scrotum or lower abdomen. The fluid is drained and the hydrocele sac is excised or everted to prevent fluid re-accumulation. The procedure is safe and effective with a low recurrence rate.
Recovery
Most patients return home the same day or after a short stay. Mild discomfort, bruising, and swelling are expected for a few days. Normal activities can generally be resumed within 1–2 weeks.
